Primary Responsibilities:

The MDS Coordinator is a Registered Nurse (RN) responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator identifies resident acuity and needs, helping to determine specific care needs and communicating needs as well as expectations to families and responsible parties. This role is responsible for completing assessments, Minimum Data Set (MDS) and care plans for all residents assigned. The MDS Coordinator will monitor the completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines. This position will advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing. The MDS Coordinator will ensure accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents. Use your Registered Nurse (RN) knowledge to initiate care plans and supporting activities that will result in best possible outcome for assigned residents as well as review resident accidents to ensure care plans reflect changing needs.

Qualifications:

  • Current Registered Nurse (RN) license

  • One year of previous experience in long term care environment

  • Knowledge of MDS and Care Plan process

  • Point Click Care software experience preferred
